Mr. David Cook , a resident of Wallaceburg , passed away on Sunday, November 6, 2016 at the age of 44 years .

David was born in Leamington, Ontario.

Dear son of Casey and Gladys den Boer and Brenda Hayward.

Kind brother of Richard (Kari), Kenneth (Michelle), Sean, Crystal and Ally.

Sadly missed by close friends Phil Forde and Leroy Lewis.

Friends and family will be received at the Eric F. Nicholls Funeral Home Ltd. 639 Elgin St., Wallaceburg, on Saturday, November 12, 2016 from 4-7 p.m.

The Funeral Service will follow in the Chapel of the Funeral Home at 7 p.m.

Cremation will follow.

As an expression of sympathy, donations to the Cerebral Palsy Foundation may be left at the funeral home.

As a living memorial, a tree will be planted in Nicholls Memorial Forest in memory of David Cook .
